- Fees due at time of service:
- Initial Appointment - $150 (Medicare Co-Pay $27.05)
- Therapy 50-55 minutes- $125 (Medicare Co-Pay $22.87)
- Medicare Annual Deductible 2022 - $233
- (You pay the first $233, then Medicare covers 80%)
- Out-of-Network insurance - I will provide a super bill for you to file after the fee is paid.
